Utility unit on south side of sidewalk has door open and it is clicking. Not sure if wind blew it down or what. Also not sure if it is electric, telephone or cable. I assume it is not a city device, but hoping you can notify correct utility company.

Erosion hole under sidewalk, potentially draining into storm sewer

1) Walked down to see the Mayfield bridge replacement this evening and witnessed numerous turn-arounds on both sides of the construction barrier. Also witnessed one car squeeze through. There is only one sign at Mayfield/Providence and many are not noticing it as they zoom through intersection at full speed.

On the east side of bridge, a friend’s neighbor counted 400 turn-arounds this morning at Mayfield Cir.

Suggestion: maybe have addition warning signs at Providence/Mayfield and at Canton St/Mayfield? Maybe even a sign in _middle_ of road (or would that be unsafe)?

2) Hoping that pedestrian / bike access can remain as long as possible.

CVS grass and landscaping is an eyesore. Not sure if this violates city code.

Pothole northbound Warsaw just before Walmart side entrance

Pothole southbound, right lane in front of Rivers Academy private school

Pothole eastbound Mansell, on eastern "box" of Colonial Center Parkway intersection.

This pothole was first reported a few weeks ago. City closed issue stating it was fixed. They must have fixed the wrong pothole. I have provided a picture this time to help.

Huge pothole on eastbound (I think it was just past front of Big Pie pizza) on left lane.

Crosswalk button and associated sign are slightly loose. This is the sign on Southwest corner of Mansell/Rt.9 that is used to request crossing from South to North side of Mansell (to Chipotle)

Bad asphalt repair after utility work. Somebody said it was the gas company, but I am not sure. This is at the exit of Mayfield Place so a lot of cats hit it. It seems to be deteriorating.

Don White Park restroom: supply pipe for men's urinal dripping water. Noticed this Friday, but forgot to report it. One drip every 10-15 seconds. Small puddle near wall.

Very small tree/branch has fallen and is encroaching on south side of roadway (between Solvay entrance and McGinnis). Not blocking roadway, but may scratch cars that are too far right.

During heavy rainfall in recent weeks, I have noticed water pooling on north side of this bridge. I know enough to slow down. Others may not see it.

Sidewalk, crosswalk, road not draining near new round-a-bout. Could be problem in winter if it freezes.

Thus is the east Crosswalk across Bates road at intersection with Mayfield Rd.

Muddy water running into roadway storm drain at construction site next to Southern Comforts Consignments

Small dead deer in left lane on eastbound Windard. I think it was just before Club House Dr.

sinkhole on south edge of road, just West of Foe Killer Creek. Between steel bridge railing and road

Backup on Windard exit ramp from 400 S. ~6PM weeknight. Light turns green and only a few cars can get through since traffic is backed up on Windward. Seems like westbound traffic on Windward should have shorter green light to allow room for those exiting 400. Heard a new lane is on way, so that is good.
